This is to guarantee the scientific and ethical soundness of each research protocol involving human participants. ETHICS CLEARANCE REQUIREMENTS (Executive Order 33, S. 2023) The researcher/s must submit the following physical documents to begin the review process of the research proposal to the OUS Center for Research and Extension: Letter of Intent addressed to the Vice President for Research, Extension, Planning, and Development (VPREPD), Dr. Anna Ruby P. Gapasin; Application Form (UREC Form 9); Accomplished Research Protocol which includes the title, significance of the study, literature review, objectives of the study, methodology and procedures, description of the study population, exclusion and inclusion criteria, data analysis, and ethical considerations. The section for ethical considerations shall state ethical issues like anticipated risks (how these will be minimized) and potential benefits; protection of confidentiality of data and privacy of the research participants; vulnerability of research participants; management of adverse events, and how the informed consent form will be obtained. (UREC Forms 10); Informed Consent Form and Assent documents for protocols involving minor participants (UREC Form 11); Research questionnaire with reliability or validity test results. In the case of an adapted research questionnaire, a copy of any document from the original author allowing the researcher to use the study tool is necessary; Curriculum Vitae of the researcher/s; and Co-authorship agreement for multiple authorship.
