Before you apply online, please make sure that you have the following files on your device or USB drive (each file size must not be more than 300 kilobytes):

• Applicant's photo (JPEG file; read photo guidelines)

• School credential from the last school attended:

  • For college undergraduates, Transcript of Records

  • For technical school/TESDA graduate and undergraduates, Transcript of Records

  • For High School graduates, Form 137-A and Form 138

  • ALS/PEPT graduates receives a Certificate of Completion, and Result, and Diploma
