Applicant's photo (JPEG file; read photo guidelines)
School credential from the last school attended:
- For college undergraduates, Transcript of Records
- For technical school/TESDA graduate and undergraduates, Transcript of Records
- For High School graduates, Form 137-A and Form 138
- ALS/PEPT graduates receives a Certificate of Completion, and Result, and Diploma
