Before applying online, please prepare the scanned, clear copies of the following items saved as JPEG

(.jpg or.jpeg, whose file size is not more than 300 kilobytes) on your computer, USB, or device.

Your 2x2 – inch colored photo with name tag
Your Grade 11 report card showing your complete name, LRN, and GWA for the first and the

second semester.
