The letter of complaint must contain the following information:

• The complete name and address (if known) of the complainant and the person(s) complained of, as well as his/her/their position(s) and office(s);

• A detailed narration of the relevant facts, demonstrating the actions orallegedly involved;

• if any; and If applicable, submit certified true copies of any documentary evidence and affidavits from witnesses.

• Certification or statement from non-forum hopping.
