What should be the content of the complaint letter?

The complete name and address (if known) of the complainant and the person(s) complained of, as well as his/her/their position(s) and office(s);
A detailed narration of the relevant facts, demonstrating the actions orallegedly involved;
if any; and If applicable, submit certified true copies of any documentary evidence and affidavits from witnesses.
Certification or statement from non-forum hopping.
