How do I file a complaint?

The aggrieved or concerned party must immediately notify the Office of the Director of Student Services or the Head of Student Services in the case of branches and satellite campuses.
The Office of the Director of Student Services must attempt to resolve all grievances and concerns through consultation and dialogue with the concerned parties.
In all cases, the ODSS will recommend that the parties go to the Office of Counseling and Psychological Services for intervention and assessment of their psychological condition.
For cases settled amicably, the OCPS shall provide recommendations or assessments to the ODSS, and cases that fail to reach an amicable settlement shall be referred to the Student Disciplinary Board for appropriate action. The OCPS shall also forward the case record and assessment to the SDB.
For cases initiated by students against a faculty member or
a non-teaching personnel:
The ODSS shall refer the complaint to the OVPAA, OVPA, or OVPBC for appropriate action. For record or referral purposes, the OVPAA, OVPA, or OVPBC must provide the ODSS with copies of its resolution(s), recommendation(s), finding(s), and/or action(s).
If the parties do not reach a settlement, the OVPAA, OVPA, or OVPBC shall refer disciplinary or administrative cases to the President to establish an investigation committee following CSC rules or to forward the case or complaint to the Office of the University Legal Counsel for appropriate action.
